How to Refer a Patient to Willow Creek Behavioral Health

If you’re a health care provider with a patient referral, we encourage you to contact us directly.
The admissions team at Willow Creek Behavioral Health is available 24/7 to support you and schedule an in-person assessment at our Green Bay facility.

Please be prepared to provide:

Patient name and date of birth
Symptoms, behaviors and a summary of the need
Known medical conditions and medications
Insurance

Discharge information is sent to continuing care providers at time of discharge by our Medical Records Department.
